Click on this link to get a 2 page patient health history form. Please bring the completed form to our office, one for each child who has an appointment.
Please arrive 10 minutes prior to your scheduled appointment time so we can complete your child's chart.
Payment in full is expected at the first visit, regardless of insurance coverage. If applicable, you will be given an itemized receipt to submit to your insurance for reimbursement. Please note, we are an out-of-network provider for all insurance plans.
At least 24 hours notice is required to change or cancel an appointment to avoid a $25.00 missed appointment fee.
