Click on this link to get a 2 page patient health history form. Please bring the completed form to our office, one for each child who has an appointment.
Please arrive 10 minutes prior to your scheduled appointment time so we can complete your child's chart.
We are a participating provider in the Delta Dental Plan. Payment is expected a the time of the first visit unless there is coverage with a Delta Plan.
At least 24 hours notice is required to change or cancel an appointment to avoid a $25.00 missed appointment fee.
