Effective July 1, 2005, all Illinois children in kindergarten, second and sixth grades are required, under the Illinois School Code, to have a dental examination.
A completed dental school form must be given to the school by May 15th of the kindergarten, second and sixth grade school years.
Make your appointment today to be sure your child has a school form for the appropriate grades. Please bring in your form received in your school information packet or ask us for one, we keep them on hand for your convenience!
